Job summary

The Colorado Department of Corrections seeks a dedicated professional to provide comprehensive pre-release re-entry services to inmates transitioning from incarceration to society. You will support offenders with transitional planning, coordinate with internal and external partners, and document progress throughout re-entry.

Qualifications

• Three (3) years of experience working directly with a hard-to-serve high-needs client population in corrections or human services, with case management including assessment, planning and coordination of community services.
• Three (3) years of corrections experience with offender management components (ICC, classifications, job placement, supervision, crisis intervention).

Responsibilities

• Conduct meetings with inmates within six months of release to identify criminogenic needs and develop transitional release plans.
• Coordinate effective release planning with multidisciplinary teams and community resources.
• Maintain documentation of offender contacts and progress in transition plans.
• Collaborate with parole staff to identify stabilization resources for transitioning parolees.
• Attend meetings with stakeholders to plan community re-entry initiatives.
