Job summary

Pueblo Community College seeks a dedicated professional for pre-release re-entry services, providing comprehensive support to offenders transitioning from incarceration to society. You will coordinate assessments, plan resources, and collaborate with multiple stakeholders to facilitate successful reintegration.

The role emphasizes evidence-based practices, crisis intervention, and strong client advocacy to ensure public safety and positive outcomes for parolees across Colorado.

Qualifications

• Three years of experience with hard-to-serve clients in correctional or human services, performing case management and assessment of risks and needs.
• Experience coordinating community services and monitoring compliance/overall supervision of cases.
• Corrections experience including offender management and related processes (ICC, hearings, placement, supervision, tutoring).

Responsibilities

• Meet inmates within six months of release to identify criminogenic needs via assessments and file review.
• Develop transitional release plans with inmates and resource coordination.
• Monitor inmate progress and revise plans as needed with status changes.
• Conduct interviews to gather information for effective release planning.
• Utilize evidence-based practices and crisis intervention as needed to support change.
• Assist offenders in applying for community services to aid parole success.
• Maintain collaboration with internal and external partners for feedback and resolution of issues.
• Document contacts and progress of transitional release planning.
• Make recommendations to multi-disciplinary teams regarding release planning.
• Maintain customer service and relationships with community partners and service providers.
• Collaborate with parole staff to identify stabilization resources for transitioning parolees.
• Attend meetings to plan community re-entry initiatives and local services.
• Complete parole plan investigations within established timeframes.
• Other duties as assigned.
