Job summary

You will perform professional case management for adult offenders, assess needs, develop reentry plans, and monitor outcomes while coordinating with security and administrative teams to ensure compliance with policies and procedures.

The role includes client instruction, data entry, and collaboration with external agencies to support community reintegration.

Qualifications

• Bachelor’s degree in a related field is required.
• 2 years of professional experience in correctional counseling/case management or human services.
• Familiarity with court orders, probation rules, and community resources preferred.

Responsibilities

• Case Management – orientate clients, assess risks, develop reentry plans.
• Deliver individual and group instruction on cognitive skills, housing, employment, and life skills.
• Maintain comprehensive files and document services provided.
• Oversee medication Dispensation in collaboration with security staff.
• Input and manage data to analyze client outcomes and program effectiveness.
• Collaborate with judicial and county personnel and represent the program at meetings.
