Job summary

Weld County is seeking a qualified Case Manager to provide professional case management for adult offenders in a Work Release facility. You will develops reentry plans, coordinate resources, and document client outcomes to support successful community reintegration.

The role involves collaboration with security and administrative staff, and delivering individual and group instruction in cognitive skills, housing, employment, relapse prevention and life skills.

Qualifications

• Bachelor's Degree in criminal justice, social work, social sciences, sociology, psychology, counseling or similarly related field.
• 2 years of professional experience in a correctional counseling/case management services position or human services related environment.
• Knowledge of courts, departments and Weld County operations; ability to navigate databases and case management systems. (Medium proficiency)
• Ability to translate/interprete English/Spanish preferred (Medium proficiency).

Responsibilities

• Orientate clients; review rules and regulations, court orders, assess risks; identify obstacles and strengths using formal and informal assessment tools.
• Assist clients in developing reentry plans into the community based on risk and needs.
• Deliver individual and group instruction in areas such as cognitive skills building, housing, employment, relapse prevention, life skills, and abilities for prosocial change.
• Maintain files and documentation of program services.
• Work with security personnel and oversee the management and dispensation of medication to clients.
• Input and maintain data to analyze outcomes of client compliance and statistical measurement; evaluate results to improve and/or expand services.
