Job summary

TASC (Treatment Alternatives for Stronger Communities) is seeking a full-time Case Manager for the Community Reentry program, serving clients statewide in Illinois. You will coordinate behavioral health services, benefits enrollment, and housing/resources to support successful reentry after incarceration.

Ideal candidates have a high school diploma or GED and CADC certification within two years, with strong communication and facilitation skills.

Qualifications

• High school diploma or GED; valid driver's license required.
• CADC certification within two years of employment may be required.
• Must pass an IDOC background check.
• Proficiency in the Microsoft Office suite.
• Ability to work inside a correctional setting with individuals convicted of serious offenses.

Responsibilities

• Assist individuals in achieving service plan goals through coordination of behavioral health, public agencies, and community resources.
• Assess, evaluate, facilitate, and monitor individuals’ progress against contractual benchmarks.
• Complete intake packets for program enrollment and participation.
• Provide referrals and crisis intervention for individuals’ identified needs upon release from prison.
• Interview individuals, review records, and confer with other professionals to identify reentry needs.
• Develops effective working relationships with internal and external partners; participate in meetings and staffing.
• Identify and refer individuals to initial treatment and behavioral health services.
• Complete and maintain all required program documentation.

Division Overview

The Community Reentry (CR) is a statewide network of clinical reentry management services with the overall purpose of increasing opportunities for successful reentry outcomes. CR provides specialized case management services to help people prepare to return to their families and communities after incarceration. CR works both inside prisons and in communities to provide reentry support, including, but not limited to, behavioral health referrals, public benefits enrollment, finding employment and housing resources and obtaining state IDs and vital records

We are currently looking for full-time – Case Manager

The position is statewide and may serve clients in any county throughout Illinois.

Starting at $44,676 - $47,650; contingent upon experience, education, etc.

This position will provide addiction intervention, AODA counseling and recovery support services to pregnant and post-partum mothers and their families. This position is responsible for bringing services, agencies, resources, and other people together to assist clients in achieving established goals. This position may also work with high-risk caseloads, co-occurring clients, families, and children.

• Assist individuals in achieving service plan goals through direct services and coordination of behavior health, public agencies, and community resources.
• Assess, evaluate, facilitate, and monitor individuals’ progress with contractual benchmarks
• Complete intake packets for program enrollment and participation
• Provides referrals and crisis intervention for the individuals’ identified needs upon release from prison
• Interview individuals, review records, and confer with other professionals to identify reentry needs
• Collaborate with internal and external partners, participate in staff meetings and monthly supervisions, and participate in integrated staffing with individuals receiving TASC services
• Identify and refer individuals to initial treatment and behavioral health services
• Complete and submit internal and external program reports
• Complete and maintain all required individual and program related documentation
• Assist clients with the enrollment in public benefit systems and obtaining vital records
• Participate in community meetings, trainings and projects including multi-disciplinary staffing, vocational-and educational-programming, institutional counseling, correctional-industries, employers, and other meetings assigned
• Develops effective working relationships with the internal and external partners
• Participate in meetings to adjust, update, or revise individual service plan
• Obtain and clinically analyze progress reports from internal and external partners to ensure
• individuals’ progress is on target with the benchmarks outlined in their service plan
• Ensure that confidential information relating to the organization, its staff, and individuals is kept confidential
• Be an excellent steward of TASC, modeling the core values of the organization

Our Ideal Candidate

• High school diploma or GED equivalent
• Valid driver's license
• CADC certification within two years of employment with TASC may be required based on contractual obligations
• Must be able to pass an IDOC Background check
• Demonstrated proficiency in the Microsoft Office suite
• Demonstrated experience in group facilitation
• Knowledge of the current ASAM placement criteria, DSM-V as it relates to substance-related disorders, and Code of Ethics for substance abuse professionals, or the ability to acquire the knowledge
• Comfort working inside a correctional setting and with persons convicted of serious violent offenses
• Flexibility in thinking and approaches to leading group discussions
• Good verbal, written, and interpersonal communication skills
• Good time management skills with a proven ability to meet deadlines
