Job summary

WestCare Foundation in Fresno, CA, seeks a Justice-Involved Pre-Release Care Coordinator to coordinate ECM services under CalAIM. You will work with correctional health staff and community providers to ensure continuity of medical, behavioral health, housing, and social services after release.

Responsibilities include outreach, eligibility verification, intake screenings, and individualized pre-release care planning with a multidisciplinary team. Travel to facilities is required.

Qualifications

• Experience in case management or care coordination.
• Experience with justice-involved populations preferred.
• High School diploma required; two-year degree preferred.
• Excellent documentation and organizational skills.

Responsibilities

• Conduct in-reach within correctional facilities to identify eligible individuals.
• Receive referrals and determine eligibility for ECM program participation.
• Conduct intake screenings and develop pre-release care plans with a care team.
• Coordinate discharge planning linking to medical and behavioral health services.
• Maintain records and track program data with WestCare’s systems.
