Job summary

Amity Foundation seeks compassionate individuals to join the ECM Pre-Release Care Management team. The role focuses on outreach, assessments, and coordinating medical, behavioral, and social services to support high-need populations and reduce recidivism.

Responsibilities include developing care plans with partners, documenting in the EHR within 48 hours, and linking clients to housing, mental health, and substance abuse resources. Local travel and bilingual skills are preferred.

Qualifications

• Bachelor’s degree in social work, psychology, public health, or related field.
• Minimum of two years’ experience in a client-related field, providing case management and community outreach.
• Equivalent combination of education and experience accepted.
• Experience with assessments, substance abuse counseling, and reentry services is a plus.

Responsibilities

• Conduct outreach to identify and engage eligible individuals in ECM services.
• Interview and assess medical, behavioral, and social needs.
• Develop, implement, and monitor personalized re-entry care plans in collaboration with partners.
• Monitor outcomes and adjust plans to meet student goals.
• Coordinate care across multiple providers and services.
• Document services in the Electronic Health Record within 48 hours; ensure compliance with policy and regulations.
• Coordinate links to mental health, housing, and substance abuse resources.
• Attend required trainings and meetings.
