Job summary

WestCare Inc is seeking a full-time Justice-Involved Pre-Release Care Coordinator based in Fresno, California. This role involves conducting pre-release care coordination services for justice-involved individuals eligible for Enhanced Care Management under the CalAIM Initiative. You will engage with multidisciplinary teams to ensure continuity of care and assist Members in their transition to community-based services.

The ideal candidate should have a strong knowledge of community resources, excellent communication skills, and the ability to manage multiple priorities. A high school diploma is required, while a two-year degree in a related field is preferred.

Qualifications

• 2 years of experience in case management, outreach, or care coordination preferred.
• Experience with justice-involved or behavioral health populations preferred.

Responsibilities

• Conduct outreach within correctional facilities.
• Develop individualized pre-release care plans.
• Coordinate pre-release discharge planning.
• Manage an assigned caseload of Members.
• Complete timely documentation and reporting.
