Job summary

Stanislaus County is seeking a Senior Community Health Worker to provide person-centered care coordination for justice-involved Medi-Cal beneficiaries during the 90-day pre-release period under CalAIM Justice-Involved. You will work inside correctional facilities, collaborate with the Medical Vendor, correctional staff, MCP care managers, and ECM providers to support continuity of care after release.
