Job summary

WESTCARE CALIFORNIA INC in Fresno seeks a Justice-Involved Pre-Release Care Coordinator to conduct pre-release care planning for ECM services, working with correctional health staff, managed care plans, and community providers to ensure a smooth transition to community-based care.

Qualifications

• Two years of case management or care coordination experience preferred.
• Experience with justice-involved populations is preferred.
• Graduation with a two-year degree in related field preferred.
• HS diploma or equivalent required.

Responsibilities

• Conduct in-reach within correctional facilities to identify eligible individuals.
• Verify eligibility and conduct intake screenings for ECM services.
• Develop individualized pre-release care plans with multidisciplinary team.
• Coordinate discharge planning and linkage to medical, behavioral health, housing, and community services.
• Maintain records in Clinical Data System and reporting systems.
• Provide culturally responsive services and maintain client engagement.
