Job summary

Wellpath / Correct Care Solutions seeks a CalAIM Case Coordinator H to support the reintegration of CalAIM-eligible individuals by coordinating reentry care plans under the RN Discharge Planner. The role includes Medi-Cal eligibility screenings and coordination with managed care plans, providers, and community partners to ensure continuity of care.

The position requires documenting in the EHR, tracking release timelines, and ensuring pre-release activities comply with CalAIM policy.

Qualifications

• Bachelor's degree in a health-related field or related area is preferred.
• Experience developing transition plans with community resources for medical, substance use, or mental illness is preferred.
• CPR certification and any contract-required certifications must be maintained.

Responsibilities

• Coordinate with Medi-Cal eligibility staff, in-reach providers, and community partners to support CalAIM Justice-Involved activation and reentry planning.
• Gather clinical and logistical information, track release dates, obtain consents, and support warm handoffs for continuity of care.
• Ensure timely documentation in the EHR and monitor release timelines to support service delivery and compliance requirements.
• Activate Justice-Involved (JI) aid codes in the state reentry portal and ensure alignment with pre-release timelines.
• Support continuity of care and CalAIM compliance under the RN Discharge Planner.
