Job summary

Alliance Health in Charlotte, NC seeks a Jail Liaison Care Coordinator to provide person-centered care transitions and care coordination for individuals detained in jail. You will work with jail behavioral health and medical staff to evaluate needs and plan a successful community release to prevent future detainment.

The role requires travel to Mecklenburg County detention facilities up to three times per week and occasional onsite meetings at Alliance Home Office.

Qualifications

• Bachelor's degree in human services or related field with required post-graduate experience in behavioral health or justice involved systems.
• Bachelor's degree in non-human service field with four years post-graduate experience in behavioral health or justice involved systems and at least one year in healthcare with integrated whole-person care.
• Master's degree in a human service field with one year post-graduate experience in behavioral health or justice involved systems.

Responsibilities

• Meet with members in jail settings and document consent to participate in care coordination and release planning.
• Obtain releases of information for coordination of care and collaborative release planning.
• Assess individuals' physical, psychological, social, environmental, and spiritual needs.
• Provide education to members and guardians about rights, options, and payer requirements.
• Collaborate with caregivers, providers, and interdisciplinary teams to inform assessments and transition plans.
• Develop and monitor transition plans addressing barriers to prevent future justice-system encounters.
• Coordinate post-release follow-up and timely access to services.
