Job summary

Coleman Health Services is seeking a Jail Navigator in Ohio to coordinate mental health services for individuals in the criminal justice system. You will manage discharge planning, collaborate with jail staff and courts, and connect clients with housing, employment, and community resources before and after release.

The role emphasizes advocacy, collaboration, and ensuring continuity of care in a correctional setting.

Qualifications

Minimum experience in case coordination for adults receiving behavioral health services.
Experience with individuals having mental health and/or substance use needs.
Willingness to obtain NPI and Ohio Medicaid number.
Valid driver's license with clean driving record.

Responsibilities

Conduct assessments, monitor symptoms, and connect clients to mental health and SUD services.
Coordinate care and discharge planning up to 30 days before/after release.
Build relationships with jail staff, courts, probation, and community providers.
Assist clients with housing, employment, transportation, and other resources.
Provide advocacy and referrals following release.
Maintain daily reports and communicate clients' status with providers.
