Job summary

Alliance Health in Charlotte, NC is seeking a Jail Liaison Care Coordinator to provide person-centered care transitions and care coordination for individuals detained. The role partners with jail behavioral health and medical staff to assess needs and ensure a successful transition to community care.

The position is full-time and hybrid, with occasional on-site reporting for meetings at the Alliance Home Office in Charlotte and travel to detention facilities up to three times per week to meet

Qualifications

• Bachelor’s degree in human services or related field with relevant experience.
• Bachelor’s degree in a non-human services field with at least 4 years’ experience in behavioral health/justice-involved systems.
• Master’s degree in human services or related field with experience in care coordination.
• Preferred certification as a care manager (NACCM, CCMC, or ACCM).

Responsibilities

• Meet with member in jail setting and document consent for care coordination.
• Complete assessments of physical, psychological, social, and environmental needs.
• Coordinate with jail staff and community providers to plan timely release.
• Develop care transition plans with goals and interventions.
• Provide education and guidance to members and caregivers about rights and resources.

Education

Bachelor's degree in Human Services

Bachelor’s degree in non-human service field with 4+ years related experience

Master’s degree in a human services field
