Job summary

Clark County Jail Services seeks a Jail Transition Coordinator Lead to oversee release-planning for incarcerated individuals with complex needs. You will coordinate services across jail medical, mental health staff and community partners to connect clients to housing, health care, and supportive resources.

You will review client needs, develop individualized release plans, document activities, and ensure efficient use of public resources while operating within a security environment.

Qualifications

• Master’s degree required with 1+ year experience with persons with legal system involvement OR Bachelor’s degree plus 2+ years of such experience OR 5 years equivalent related professional experience.
• Proof of credentials will be required.
• Degree must be from an institution with national recognition; transcripts required at hire. A degree is preferred or equivalent combination for consideration.

Responsibilities

• Coordinate direct services for incarcerated people with acute and complex behavioral health needs.
• Develop collaborative individualized release plans and coordinate access to resources (mental health, housing, health care, etc.).
• Work with corrections officers, jail medical and mental health staff and community partners.
• Review client needs, document activities and ensure efficient use of public resources.
• Engage with internal and external partners to facilitate connection to community-based services.
• Operate within a secured jail environment and align with mission and values.
