Job summary

Alliance in Charlotte, NC is hiring a Jail Liaison Care Coordinator to provide person-centered care transitions and care coordination for individuals detained, collaborating with jail behavioral health and medical staff to plan successful release into the community.

This is a full-time hybrid role; while onsite work is not routine, the candidate must report to the Alliance Home Office for meetings and travel to Mecklenburg County detention facilities up to three times per week.

Qualifications

• Must meet NC’s Qualified Professional requirements per 10A-NCAC 27G.0104.
• 2+ years post-graduate experience in behavioral health or justice-involved systems if holding a Bachelor in human services.
• 4+ years post-graduate experience in behavioral health with healthcare exposure if using a non-human service bachelor’s.
• Proficient in Microsoft Office applications.

Responsibilities

• Meet with member in-person in the jail setting.
• Document consent to participate with care coordination and release planning.
• Obtain releases of information for coordination of care and collaboration for successful release.
• Complete an assessment of physical, psychological, social, environmental, and spiritual needs.
• Provide education to members and guardians regarding rights, options, and payer requirements.
• Travel to Mecklenburg County detention facilities up to three times per week to meet with members.
