Trillium Health Resources is seeking a Re-Entry Care Coordinator to support individuals transitioning between care settings, especially justice-involved individuals preparing for release. You will develop transition plans, coordinate with providers, and address barriers to ensure stable transitions.
The role emphasizes collaboration, care coordination, and travel within NC counties, with remote work options for many roles.
Qualifications
- Must meet Qualified Professional (QP) status per NC requirements with MH/DD/SA experience.
- RN licensed in NC with MH/DD/SA experience (4 years minimum) or
- Master’s in human services with 1 year MH/DD/SA experience.
- Bachelor’s in human services with 2 years MH/DD/SA experience or similar.
- Bachelor’s in non-human-services with 4 years MH/DD/SA experience or similar.
Responsabilités
- Coordinate care transition activities for recipients on the High Priority Re-entry list.
- Develop and implement transition plans based on individual needs.
- Collaborate with providers and partners to align services during transitions.
- Monitor transitions and intervene to resolve delays or barriers.
- Facilitate service connections and ensure continuity post-transition.
