Qualifications

• Experience delivering recovery support or care coordination.
• Excellent interpersonal and communication skills.
• Ability to navigate healthcare, social services, and community resources.

Responsibilities

• Collaborate with a multidisciplinary care team to ensure high-quality, member-centered care.
• Build trusted relationships with members to support recovery goals.
• Identify member goals, provide coaching, and engage in recovery activities.
• Support re-engagement in treatment through outreach.
• Orient new members to the Groups care model during transition to care.
• Provide in-person, virtual, and telephonic support as needed.
• Maintain a shared caseload and remove barriers to care.
• Coordinate care pathways for high-risk or special populations.
• Link members to healthcare, behavioral health, social services, and community resources.
• Assist with navigating insurance, housing, transportation, and other needs.
• Manage referrals, ROIs, and documentation to facilitate transitions.
• Act as a community connector and advocate to reduce stigma.
