Job summary

Groups is seeking a Recovery Support Coordinator to work with a defined panel of members, coordinating care across clinical, social, and community settings. You will provide recovery support services, engage members, and help remove barriers to care within our evidence-based model.

Responsibilities include collaboration with clinicians, outreach to reengage members, and ensuring timely documentation in the EHR.

Qualifications

• High school diploma required; related associate degree preferred.
• Minimum two years in case management or care coordination.
• Experience with substance use or mental health challenges preferred.
• Reliable internet and ability to work remotely if needed.

Responsibilities

• Collaborate with a multidisciplinary care team to ensure high-quality, member-centered care.
• Build trusted relationships to support recovery goals via motivational interviewing.
• Assist members in identifying goals and providing coaching and coping support.
• Support re-engagement in treatment through intensive outreach.
• Coordinate care pathways and assist with insurance, housing, transportation.
• Manage referrals and documentation to facilitate smooth transitions of care.
