Job summary

You will provide outreach, coaching, and navigation through health, housing, transportation, and insurance, with options for in-person, virtual, and phone-based support.

Qualifications

• High school diploma required; related degrees preferred.
• Minimum two years of experience in case management or care coordination.
• Experience with Electronic Health Records (EHR) and navigating health care systems.
• Valid driver’s license and access to a vehicle may be required for community travel.

Responsibilities

• Provide recovery support and coordinate care across clinical, social, and community settings.
• Assist members with treatment engagement and navigating insurance, housing, transportation.
• Deliver in-person, virtual, and telephonic support based on regional needs.
• Maintain shared caseload and ensure coordinated service delivery.
• Engage in community outreach and education as needed.
