Job summary

Groups is seeking a Recovery Support Coordinator in Houlton, ME to support 160–180 members with health-related social needs, treatment engagement, and coordinated care across clinical, social, and community settings.

This on-site role reports to the Recovery Operations Supervisor and involves outreach, coaching, care navigation, and collaboration with counselors, prescribers, and other team members to help members achieve recovery goals.

Qualifications

• High school diploma required; associate degree preferred.
• State-specific requirements vary; MHRT/C or CHW certification may be required in Maine.
• Two years of experience in case management or care coordination preferred; experience with SUD/mental health preferred.
• Experience with Electronic Health Records (EHR).
• Reliable internet access and strong WiFi if working remote.
• Valid driver’s license and access to a vehicle if role includes community travel.

Responsibilities

• Collaborate with a multidisciplinary care team to ensure high-quality, member-centered care.
• Build relationships to support recovery goals using empathy and motivational interviewing.
• Assist members in identifying goals and engaging in recovery activities.
• Support reengagement in treatment through outreach.
• Introduce new members to the care model and support transitions to care.
• Provide in-person, virtual, and telephonic support.
• Maintain a shared caseload and coordinate service delivery.
• Navigate insurance, housing, transportation, and other resources.
