Qualifications

• High school diploma required; associate degree preferred.
• Minimum two years in case management or care coordination.
• Experience with substance use or mental health populations preferred.
• Reliable internet and WiFi for remote work.

Responsibilities

• Provide recovery support and coordinate cross-disciplinary care.
• Build relationships and motivate members toward recovery.
• Assist with goal setting and coaching.
• Navigate insurance, housing, transportation, and available resources.
• Manage referrals, ROIs, and documentation.
• Deliver in-person, virtual, and telephonic support.
• Participate in interdisciplinary team meetings.
