Job summary

High Point & Affiliated Organizations seeks a Recovery Support Navigator to deliver client-centered CBRS services, coordinate with outreach units, and maintain thorough documentation. The role requires a Bachelor's Degree, a valid driver's license, and active collaboration with diverse partners to support recovery goals.

The candidate will participate in trainings, ensure HIPAA compliance, and coordinate monthly regional activities while fostering community connections and quality care.

Qualifications

• Bachelor’s Degree in Psychology or related field or case management experience.
• Valid driver’s license and ownership of a car.
• Medical Insurance, Vision Insurance, Long & short term disability.

Responsibilities

• Provide client-centered case management CBRS services as indicated in needs assessment/service plan.
• Coordinate with regional outpatient units for client education and CBRS sign-ups.
• Maintain safety and security of client/staff at all times.
• Accurately document needs assessments, service plans, encounter notes, and service delivery reports.
