Job summary

Gilead Community Services is seeking an attentive Case Manager for its Middletown area adult residential program. You will work Tue–Fri 7:30 AM–3:30 PM and Sat 7:00 AM–3:00 PM (40 hours/wk, benefits).

Responsibilities include intake, engagement, assessment, planning, case management, referrals, and discharge, with timely documentation. The role requires a Bachelor’s in a human service field (Master’s preferred) and one year of clinical experience.

Qualifications

• Bachelor's degree in a human service field required; Master’s degree preferred.
• One year of clinical experience.
• Valid driver’s license and automobile insurance in good standing.
• Reliable use of personal vehicle as needed.
• Experience with co-occurring psychiatric and substance use disorders preferred.

Responsibilities

• Provide clinical case-management services and documentation in a timely manner.
• Deliver recovery-oriented counseling and advocacy with attention to dignity and respect.
• Support rehabilitation activities across nutrition, clothing, basic care, spirituality, and financial management.
• Monitor outcomes and ensure data reporting accuracy and compliance.
• Complete assessments and treatment planning documents on time, in both electronic and paper forms.
• Act with compassion, integrity, and ethics.
