Job summary

Gilead Community Services, Inc. in Middletown is seeking a Clinical Case Manager to join our multi-disciplinary supported apartment program staff, providing case management, advocacy, educational, and supportive services to adults with major mental illness and co-occurring substance use disorders.

The role includes intake, engagement, assessment, treatment planning, and timely documentation, following DMHAS standards.

Qualifications

• Bachelor’s degree in a human service field.
• One year of clinical experience.
• Must maintain and provide proof of valid driver’s license and automobile insurance in good standing.
• Reliable use of personal vehicle as needed.
• Experience with co-occurring psychiatric and substance abuse disorders preferred.

Responsibilities

• Provide case management, advocacy, educational, and supportive services.
• Carry out intake, engagement, assessment, treatment planning, case management, referral, and discharge.
• Complete assessments and treatment planning documentation in a timely manner.
• Provide recovery-oriented clinical case-management services with dignity, respect, and inclusion of family and other supports.
