Gilead Community Services is seeking an attentive and recovery-oriented Clinical Case Manager for the Middletown area. This full-time role (40 hours weekly) covers an evening and alternating weekend schedule within the Mental Health Intensive program for women, handling intake through discharge and ensuring timely documentation.

The position requires a Bachelor’s degree in a human service field (Master’s preferred), at least one year of clinical experience, and a valid driver’s license with

Qualifications

• Bachelor’s degree in a human service field (Master’s preferred).
• One year of clinical experience.
• Must maintain and provide proof of valid driver’s license and automobile insurance in good standing.
• Experience with trauma treatment and co-occurring psychiatric and substance use disorders preferred.

Responsabilités

• Provide clinical knowledge regarding severe mental health and substance use disorders.
• Collaborate with the treatment team to manage clients’ mental health and assess risks.
• Develop and implement recovery-oriented case-management plans with family and supports.
• Provide skills training in daily living, nutrition, financial management, and symptom management.
• Monitor outcomes and ensure program compliance through timely data reporting.
• Complete assessments, treatment plans, and documentation accurately and promptly.
• Demonstrate compassion, integrity, and ethics in all interactions.
