About the position:
The Clinical Case Manager for Gilead’s Middletown area adult supported apartment program works M, T, W, Th, 3p-11p, Every Other Weekend 3p-11p providing case management, advocacy, educational, and supportive services in a program serving adults with major mental illness and co-occurring substance use disorders. The individual in this role carries out all phases of clinical services including intake, engagement, assessment, treatment planning, case management, referral, and discharge. This role also requires the timely completion of required documentation including assessments and other treatment planning.
This is a union position and pays $20.83 per hour.
