Job summary

Lifeline Connections in the United States is seeking a Mental Health Case Manager to provide community-based behavioral health, care coordination, and re-entry support. You will develop strength-based Re-Entry Care Plans, collaborate with clinical teams, and ensure access to housing, medical care, and employment supports while maintaining compliant documentation.

The role requires a Bachelor’s degree in a related field, state credentialing, and at least one year of relevant experience.

Qualifications

• Bachelor's degree in Behavioral Health, Psychology, Social Work, Human Services, Counseling, or a closely related field preferred.
• Washington State Department of Health credential required or ability to obtain and maintain required credentialing.
• Minimum of one year of paid or volunteer experience working with individuals experiencing mental health, substance use, co-occurring disorders, or related behavioral health conditions.
• Working knowledge of behavioral health recovery principles, trauma-informed care, and participant-centered service delivery.
• Familiarity with Washington State behavioral health regulations, including RCW 71.05 and WAC 246-341.
• Substance Use Disorder Professional Trainee (SUDPT) credential (preferred).
• Experience providing re-entry, care coordination, case management, or community behavioral health services.

Responsibilities

• Under supervision, provides community-based behavioral health, care coordination, and re-entry support services to participants.
• Develop and monitor individualized Re-Entry Care Plans that are strength-based, participant-centered, and culturally responsive.
• Regularly evaluate participant progress and adjust interventions to support recovery outcomes.
• Coordinate access to behavioral health treatment, medical care, housing resources, transportation, and employment supports.
• Facilitate referrals and ensure continuity of care across providers and systems.
• Collaborate with multidisciplinary treatment teams and stakeholders to support participant outcomes.
• Maintain timely, accurate documentation in accordance with HIPAA and agency policies.
• Develop and maintain relationships with community agencies and referral partners.
