Job summary

The Salvation Army’s Hope House in San Francisco is seeking a Case Manager to support a 60-bed shelter. You will provide case management and recovery-focused services to adults in transition, coordinating assessments, plans, and referrals to housing and treatment services.

You will monitor progress, maintain records, and participate in meetings while promoting participant empowerment and self-sufficiency. A driver license and CPR/First Aid are required.

Qualifications

• Minimum of two years of experience providing social services to marginalized populations.
• Minimum of one year experience in case management, peer mentorship or related supportive roles.
• Experience addressing substance abuse, mental illness, and barriers to self-sufficiency.
• High school diploma or equivalent required.

Responsibilities

• Manage an assigned caseload up to 25 participants.
• Conduct intake interviews and assessments.
• Create Individual Transition Plans with goals and interventions.
• Coordinate with partner organizations and referral agencies.
• Monitor progress and reassess needs to keep plans relevant.
• Educate and empower participants for self-sufficiency.
• Respond to crisis situations and maintain professional boundaries.
• Maintain participant records in the Salvation Army databases.
• Attend staff and program meetings and trainings.
• Coordinate with service providers to meet holistic participant needs.
