Job summary

The Salvation Army Hope House is seeking a Case Manager in San Francisco to support adults in recovery within a 60-bed shelter. You will manage a caseload, conduct assessments, and develop individualized transition plans to move participants toward housing and treatment options.

You will collaborate with partners, maintain records, and participate in trainings. A strong background in social services and bilingual Spanish abilities are preferred.

Qualifications

• Minimum of two years of social services experience with marginalized populations.
• Minimum of one year in case management, mentorship, or group facilitation.

Responsibilities

• Manage caseload of up to 25 participants.
• Conduct intake interviews and assessments to identify needs and risks.
• Develop Individual Transition Plans with goals and interventions.
• Coordinate with partner organizations and referrals.
• Maintain participant records in the database and ensure accurate reporting.
• Attend staff and program meetings and trainings.
• Provide group facilitation and support to program participants.
• Assist with special events and transportation as needed.
• Support program operations as staffing requires.
