Qualifications

• Requires a bachelor’s degree in Human Services or related field.
• Requires Service Coordination Certificate.
• Extensive travel required.
• Requires valid state driver’s license and auto insurance.

Responsibilities

• Develops relationships with the individual, family and other important people identified with consent of the individual served.
• Promotes service planning with consumer, family and other providers to develop and implement a comprehensive plan.
• Reviews cases, meets with families, treatment team, advocates, and staff, attending staffing as needed.
• Maintains ongoing documentation and adheres to regulatory requirements for service coordination.
• Convenes and facilitates service planning meetings with consumer and family participation.
• Engages in community resources discovery and linkage for consumer needs and preferences.
• Provides culturally competent services considering consumer background and needs.
• Ensures crisis planning and on-call coverage when required.

Education
