Job summary

DePaul Edgerton Square is seeking a dedicated Residential Services Coordinator to support residents in a CSO/SRO program. You will help develop individualized service plans and coordinate recovery-oriented services to enhance autonomy and quality of life.

The role requires strong advocacy, direct support, and collaboration with health professionals, with duties spanning intake tours, daily living skills, and medication supervision.

Qualifications

• Bachelor’s degree in Human Services with any related experience.
• Associate degree in Human Services with at least 3 years of experience.
• High School diploma with at least 5 years in Human Services field.
• Clean/valid New York State driving record; must be at least 18 years old.

Responsibilities

• Provide tours and intake input to admissions teams for potential residents.
• Develop comprehensive, person-centered service plans and update them regularly.
• Conduct quarterly plan reviews with residents and support teams.
• Deliver recovery-oriented supports: skills training, health education, and counseling.
• Monitor and document resident progress; maintain accurate files and records.
• Schedule annual physicals and referrals, and arrange transportation when needed.
• Collaborate with Health Home Care Manager and community providers for continuity of care.
• Support daily living skills, clothing and personal care planning, and family/community engagement.
• Assist Medication Coordinators with medication supervision and related duties.
• Ensure safety, confidentiality, and professional conduct; participate in staff meetings.
