Job summary

East House is seeking a Service Coordinator to partner with individuals in recovery to access housing, treatment, and community supports. You will develop individualized service plans and ensure continuity of care through strong advocacy and collaboration with providers.

The role requires driving in the community, supporting clients at home, and maintaining detailed documentation while promoting recovery, independence, and quality of life.

Qualifications

• Provide client-centered service plans and goal setting with clients.
