Qualifications

• Service Coordination Certificate is required.
• Valid state driver’s license and means of reliable transportation.
• Extensive travel may be required by role.
• Experience with coordinating services and family collaboration is expected.

Responsibilities

• Develops relationships with individuals, families and treatment teams.
• Coordinates assessment, service planning and ongoing care.
• Reviews cases and attends staffing in or out of the office.
• Ensures access to cross-system services and crisis planning.
• Maintains accurate service documentation and compliance with regulations.
