Qualifications

• Two years of experience providing care management for individuals with behavioral health needs.
• Current Motor Vehicle Driver's license required.
• Must be CPR/First Aid Certified or eligible.

Responsibilities

• Provide service delivery in accordance with DMHAS standards.
• Document assessments and recovery plans thoroughly.
• Facilitate yearly reassessments and develop recovery plans.
