Community Access seeks a Service Coordinator to provide engagement and comprehensive support to residents in our supported housing programs in New York City. You will help individuals maintain housing, pursue recovery, access community resources, and explore education or vocational opportunities.

Work is person-centered, with responsibilities including group activities, timely documentation, and collaboration with tenants and staff.

Qualifikationen

• High school diploma or GED required; bachelor’s degree preferred.
• Ability to prioritize and meet deadlines.
• Excellent oral and written communication skills.
• Ability to work independently and as part of a team.
• Fingerprinted and cleared through a background check.
• Bilingual Spanish-speaking preferred, other languages beneficial.

Aufgaben

• Facilitate a minimum of four groups per month with diverse topics.
• Complete assessments and service plans on time.
• Document service notes within 48 hours of meetings.
• Ensure monthly in-apartment visits for tenants on caseload.
• Provide direct service work totaling around 40 hours monthly.
