Community Mental Health Affiliates, Inc. in New Britain, CT, is seeking a full-time Case Manager for the Supported Housing program. The role is based at 233 Main St., New Britain, CT 06051, with day shifts, 8:00 am–4:00 pm or 8:30 am–4:30 pm.

The position focuses on client recovery goals, coordinated care, and community integration. The case manager will maintain caseloads, deliver education and life-skills coaching, develop treatment plans, collaborate with teams, and ensure timely

Qualifications

• Bachelor’s Degree in a mental health related field preferred or equivalent experience.
• Valid Connecticut driver’s license and a reliable vehicle.
• 1+ year of case management or related experience.
• Proficient with Microsoft Office Suite.
• Experience with Electronic Medical Records (EMR) systems.

Responsibilities

• Maintain caseload and provide direct care including education, skill building, recreation and environmental monitoring.
• Develop and review individualized treatment plans with clients.
• Assist with finances, entitlements and daily living activities.
• Coordinate with CMHA teams and external providers; provide transportation as needed.
• Document progress notes, treatment plans and related CMHA documentation per policy.
