Qualifications

• Bachelor’s Degree in a mental health related field preferred or equivalent experience.
• Caseload management experience preferred.
• Experience with co-occurring conditions is beneficial.
• Proficiency in MS Office and EMR systems expected.

Responsibilities

• Maintains appropriate caseload as determined by CMHA and funder.
• Provides direct care through education, skill building, recreation, and monitoring.
• Teaches life skills to help clients live independently in the community.
• Develops and reviews individualized treatment plans with clients.
• Ensures timely documentation per CMHA policies and procedures.
• Supports with medication administration if applicable.
• Collaborates with CMHA teams and coordinates with external providers.
• May provide transportation as needed.
• Performs other related duties as assigned.
