Job summary

Community Mental Health Affiliates, Inc. is seeking a full-time Case Manager for the Greater Waterbury Care Center to support individuals with mental health, medical, and co-occurring needs.

The role emphasizes person-centered recovery goals and community integration, with a set weekday schedule at 255 Bank St., Waterbury, CT. You will develop treatment plans, document progress, and coordinate services across teams while ensuring confidentiality and compliance with policies.

Qualifications

• Bachelor's degree in a mental health related field preferred or equivalent experience.
• Experience in case management or related field with solid understanding of mental health and co-occurring conditions, minimum of one year.

Responsibilities

• Maintains caseload as determined by CMHA and funder.
• Provides direct care through education, skill-building, and monitoring in clients' environments.
• Develops and reviews individualized treatment plans with clients.
• Ensures timely documentation according to CMHA policies (progress notes, treatment plans, authorizations).
• Collaborates with CMHA teams and external providers to coordinate services and transportation as needed.
