Job summary

Community Mental Health Affiliates, Inc (CMHA) is hiring a full-time, 1st shift Case Manager for its Supported Housing program in New Britain, CT. The role focuses on empowering clients with mental health and co-occurring needs through person-centered recovery goals and community integration.

Responsibilities include maintaining caseloads, providing direct care and education, coordinating services, and documenting progress.

Qualifications

• Bachelor's degree in a mental health related field preferred or equivalent experience.
• CT driver's license and reliable vehicle; certification in MANDT, First Aid, and CPR.
• Experience in Case Management or related field; 1+ year preferred.
• Experience with Microsoft Office and EMR systems.

Responsibilities

• Maintains caseload as determined by CMHA and funder.
• Provides direct care through education, skill-building, and supervision of activities.
• Teaches life skills and supports daily living activities and housing needs.
• Develops individualized treatment plans and documents progress per policy.
• Interacts with external providers to coordinate services and may provide transportation.
