Qualificações

• Member of a multi-disciplinary team delivering psychosocial rehabilitation and recovery-focused services.
• Identifies client needs across medical, psychological, vocational, social and daily living domains.
• Coordinates recovery plans, schedules team meetings, and monitors service integration.
• Supports individuals and families in various formats to promote independence and resiliency.
• Maintains strict documentation and reporting per agency policies and DHS timelines.

Responsabilidades

• Works with assigned participants to develop a self-directed recovery plan.
• Gathers, coordinates, and schedules recovery-based team meetings.
• Transfers information, reviews progress, and addresses barriers.
• Advocates for clients within the community and connects to resources.
• Attends meetings, trainings, and updates professional knowledge.
• Performs other related duties as required.

Formação académica

Bachelor's degree in social work or related field
