Job summary

Journey Mental Health Center seeks a Case Manager to provide a wide range of culturally sensitive, recovery-oriented clinical services and case management in Dane County. The role focuses on assessments, treatment planning, referrals and coordination with treatment providers.

Preference is given to those with Substance Use Disorders experience and related credentials. The position requires a Bachelor’s degree in a related behavioral health field, a valid driver’s license, and willingness to use

Qualifications

• Bachelor’s degree in a related behavioral health field required.
• Experience in related behavioral health field preferred.
• Knowledge of recovery-oriented and trauma-sensitive practices.
• Ability to obtain and maintain relevant licensure/certification when applicable.

Responsibilities

• Perform accurate assessments including OWI and SUD considerations.
• Develop treatment plans, provide referrals, and coordinate case management.
• Complete timely clinical documentation and records updates.
• Collaborate with an interdisciplinary team and community providers.
