This role emphasizes strengths-based planning, coordinated care, documentation compliance, and collaboration with providers, families, and community partners to support recovery and independence.

Qualifications

• Bachelor's degree in Social Work, Psychology, Rehabilitation, Human Services, or related field.
• Valid driver's license, reliable transportation, and auto insurance.
• Strong interpersonal, communication, organizational, and time management skills.
• Ability to work independently while collaborating with a multidisciplinary team.
• Basic computer proficiency and experience with electronic health records.

Responsibilities

• Build meaningful relationships with individuals and families and provide recovery-focused support.
• Conduct strengths-based assessments and develop personalized service plans.
• Coordinate care with health providers, hospitals, families, and community partners.
• Connect clients with housing, healthcare, employment, transportation, and other resources.
• Advocate with healthcare providers, courts, and government agencies as needed.
• Monitor progress and adjust plans to support recovery and independence.
• Maintain accurate documentation meeting agency, Medicaid, and regulatory requirements.
• Collaborate through team meetings, supervision, training, and professional development.
