Job summary

Commonwealth of Massachusetts seeks a Case Manager I to provide assessment, outreach, and service coordination for DMH authorized adults. The role emphasizes recovery, trauma-informed care, and person-centered choices while ensuring dignity and independence in service plans.

Responsibilities include developing ISP, coordinating housing, employment support, and clinical services, and maintaining detailed documentation.

Qualifications

• Ability to work within DMH mission and communicate effectively, both orally and in writing.
• Knowledge of mental health issues and psychosocial needs of adults with serious illness.
• Ability to work independently, prioritize tasks, and manage time.
• Experience building supportive relationships with clients and providers.
• Familiarity with crisis intervention and risk management.

Responsibilities

• Assess needs and coordinate services to develop ISP per DMH rules.
• Engage clients with outreach, housing and employment support, and treatment participation.
• Schedule home visits and monitor safety of environments.
• Prepare progress notes and maintain EHR documentation.
• Lead meetings to review and modify ISP with clients and providers.
• Coordinate transitions across housing and care settings.
• Ensure services are in place and compliant with confidentiality rules.
• Identify risk factors and develop safety plans with supervisors and staff.
• Respond to crises and arrange transportation as needed.
• Travel to client sites and maintain collaboration with community partners.
