Job summary

PMHCC, Inc. in Philadelphia, PA is seeking a BE Case Management Specialist to provide community-based case management for individuals with complex needs within the mental health system.

The role leads in areas such as housing, vocational and entitlement services, coordinating with primary care and other service providers to improve access and independence. Strong communication and teamwork are essential.

Qualifications

• Bachelor’s Degree in sociology, social work, psychology, gerontology, anthropology, political science, history, criminal justice, theology, counseling or education with two years related experience.
• Bachelor’s Degree in nursing with 3 years mental health direct care experience; be a Registered Nurse (RN).
• Expertise in behavioral health with one or more of the following required: criminal justice system, substance abuse, vocational, geriatric, MR, housing or entitlement issues.

Responsibilities

• Assess individual’s strengths, needs and wants using standardized instruments and develop client-centered plans.
• Coordinate with multidisciplinary team to ensure service coordination and linkage with primary care.
• Periodically complete Environmental Matrix and determine level of care needed.
• Monitor progress toward goals, update plans and document activities per agency forms.
• Maintain community resource files and provide training on accessing housing, vocational and entitlement resources.
