Job summary

Mental Health Partnerships is seeking a Case Manager Trainee to support participants in Philadelphia with recovery goals, housing resources, and access to services. The role requires in-person client visits, some remote work one day per week, and onboarding through a 90‑day training period to prepare for a full Case Manager position.

The position offers competitive pay of $21.16 per hour, full-time hours, and benefits including PTO, life/disability insurance, and training opportunities.

Qualifications

  • High School diploma with 12 social science credits required; bachelor degree preferred.
  • Minimum 2 years in a behavioral health work environment preferred.
  • Must have a BHTEN certification or obtain one within 1 year of hire.
  • Must possess a valid driver''s license and, if applicable, qualify for a prison ID.
  • Demonstrated knowledge of the local Mental Health System and ability to work with diverse teams.

Responsibilities

  • Collaborate with participants to create self-directed goals for recovery and independence.
  • Assist with educational, employment, social goals and accessing benefits, housing options, and health services.
  • Attend community activities and document progress in the electronic health records system.
  • Support program operations and other duties as assigned to fulfill MHP's mission.

Job description

Case Managers provide services to participants living in the community with persistent mental health challenges, in addition to individuals with co-occurring conditions. After assessing the participants' needs the Case Manager helps the participant with obtaining resources and services in the community, in a partnership that focuses on recovery and the achievement of the participant's goals for community living. Case Manager Trainees receive extended training across 90 days to be able to successfully transition as a Case Manager with Mental Health Partnerships.

Mental Health Partnerships offers opportunities across the following programs and locations:

ACCESS I & ACCESS II: Supporting individuals experiencing housing insecurity.

Location: 2709 N. Broad St, Philadelphia, PA 19132

Forensics: Supporting participants who have experienced the criminal justice system.

Location: 2950 Parkside Ave, Philadelphia, PA 19131.

SHARE TCM: Supporting participants who experienced an increase with inpatient hospitalization.

Location: 4950 Parkside Ave, Philadelphia, PA 19131.

The Case Manager Trainee is a full-time position, Monday - Friday, 8:30 AM - 5:00 PM. The rate of pay is $21.16 / hr. After 90 days, the successful candidate, meeting expectations and completing their onboarding, will be promoted to Case Manager. The Case Manager is required in-person participant visits and office days and occasional opportunity to work from home once a week based on program needs. WHAT YOU'LL DO
  • Collaborate with participants to create self-directed goals to assist them in their path to recovery and self-sufficiency.
  • Support participants with educational, employment, and social related goals, benefit/medical resources, mental health and substance use services, budgeting skills and exploring housing options in accordance with their support plan.
  • Attend socialization activities, such as attending community events, recovery learning centers, 12 step meetings, etc.
  • Collaboratively document participant progress in the electronic health records system.
  • Other duties as assigned to fulfill MHP's mission.
REQUIREMENTS
  • A High School diploma with a minimum of 12 social science credits required, a Bachelor degree preferred with major course work in Sociology, Social Work, Psychology, Gerontology, Anthropology, Political Science, History, Criminal Justice, Theology, Nursing, Counseling or Education preferred.
  • Minimum of 2 years of experience in a behavioral health work environment preferred.
  • Must have a BHTEN certification or be able to obtain one within one year of hire date.
  • Must possess a valid driver's license.
  • If applicable, must be able to qualify for and obtain a prison ID.
  • Demonstrated knowledge of the local Mental Health System.
  • Must possess basic computer skills to perform job duties including desktop computing, e-mail, timesheet management, electronic health records, internet research, etc. and the ability to learn software applications relevant to your position.
  • Must have access to a car to use for work on a daily basis.
  • Must have automobile insurance coverage at the state requirement levels and current state registration.
  • Ability to work independently and to carry out assignments to completion within parameters of instructions given, prescribed routines, and standard accepted practices.
  • Must be able to work under pressure and meet deadlines and billable unit requirements, while maintaining a positive attitude and providing exemplary customer service.

PHYSICAL DEMANDS
  • Sit/stand at a desk with ergonomically appropriate equipment.
  • 80% of a Targeted Case Manager's time is spent in community settings and/or outreach. Targeted Case Managers must be able to travel within the community that may not be accessible: walk, drive agency vehicles, and/or take public transportation depending on the needs of the program.
