Mental Health Partnerships is hiring a Targeted Case Manager in Philadelphia. The role requires coordinating community-based services for individuals with persistent mental health challenges, and co-occurring conditions.
It includes helping secure resources, housing, benefits, and health services, with in-person and occasional home visits. The position is full-time with a 8:30 AM–5:00 PM schedule (Mon–Fri) and a hybrid option after 90 days.
Targeted Case Managers provide services to participants living in the community with persistent mental health challenges, in addition to individuals with co-occurring conditions. After assessing the participants' needs the Targeted Case Manager helps the participant with obtaining resources and services in the community, in a partnership that focuses on recovery and the achievement of the participant's goals for community living.

ACCESS I & ACCESS II: Supporting individuals experiencing housing insecurity.
Location: 2709 N. Broad St, Philadelphia, PA 19132
Forensics: Supporting participants who have experienced the criminal justice system.
Location: 2950 Parkside Ave, Philadelphia, PA 19131.
SHARE TCM: Supporting participants who experienced an increase with inpatient hospitalization.
Location: 4950 Parkside Ave, Philadelphia, PA 19131.
The Targeted Case Manager is a full-time position, with a schedule of Monday - Friday, 8:30 AM - 5:00 PM (with the opportunity to create a flexible schedule with manager's approval), and a starting rate of $23.00/hour. This is a hybrid work from home position (after 90 days), with required in-person participant visits and office days and occasional opportunity to work from home once a week based on program needs.
