Job summary

Mental Health Partnerships seeks a Targeted Case Manager – D&A Specialist to support participants with substance use and mental health challenges in the community. Includes in-person visits, some remote work, and a $24.00/hour rate.

Based at 414 East High Street, Pottstown, PA with Monday–Friday hours and flexible scheduling with manager approval. The role emphasizes recovery, coordination with a case management team, and facilitating access to resources, housing, and services.

Qualifications

• Bachelor’s degree in sociology, social work, psychology or related field.
• Minimum 2 years in behavioral health with focus on substance use.
• Must have valid driver’s license and access to a car.

Responsibilities

• Assess substance use to determine treatment and recovery needs.
• Develop individualized treatment plans with the team.
• Link participants to substance use services and monitor progress.
• Provide consultation to case managers on engaging individuals.
• Coordinate weekly Drug and Alcohol Recovery group.
• Maintain accurate documentation in EHR during services.
