Job summary

Community Medical Centers, Inc. is seeking a Care Assistant to support clients from admission through residential stay and transition.

You will work with a multidisciplinary team to ensure a safe, recovery-oriented environment, assist with admissions, intake, vital signs, medication storage, daily programming, and medical coordination. Responsibilities include documenting in the EHR, collaborating with nursing and behavioral health staff, and assisting with meal preparation and transportation

Qualifications

High school diploma or equivalent.
EMT training preferred; BLS/CPR certification or ability to obtain before hire.
Valid California driver's license and reliable transportation if travel between sites.

Responsibilities

Coordinate client admissions and complete intake documentation.
Welcome and orient clients to the program, schedule, and expectations.
Obtain and document vital signs; notify licensed staff of changes.
Support medication reconciliation and storage; assist with refills with providers.
Maintain accurate documentation in the electronic health record.
Assist with daily programming, life skills, and coordination of medical services as needed.
Travel between Community Medical Centers locations as assigned; assist with meal prep and procurement.
