Responsibilities

• Ensure patients receive comprehensive services including intake, assessment, discharge planning and warm handoffs.
• Provide advocacy, recovery and wellness support to patients.
• Assist in identifying patient needs and applying evidence-based approaches.
• Engage patients in the living room process and build collaborative relationships for discharge planning.
• Document discharge planning components accurately in the EMR and support quality initiatives.
